External Facilities Request for Sovereign Grace Fellowship

Please complete this form if you would like to request the use of the facilities for an event.  Please complete and sign the request (preferably 30 days in advance). Note: Request is not finalized until approved by the Deacon Board. Specify all necessary needs, including room requirements and equipment, as well as preparation, usage, and cleanup times.

General Responsibilities

Please clean up after the event, returning any used items to their proper locations. Inform the event coordinator of any supplies that need to be ordered to replenish stock levels following your event.

Section 9:

Fees: 

Fees for the facility rental and associated services (A/V operator fee, etc...) will be determined using a sliding scale based on occupancy. All fees will be discussed during the meeting with our event coordinator. All fees must be paid in advance before approval is final. The cleaning fee is a $300 flat fee.

Acknowledgment of Responsibilities

I have read and agree to the attached stated requirements of Sovereign Grace Fellowship, and understand that I assume liability for property damage. I also understand that Sovereign Grace has the reserved rights to alter or terminate approval of requested usage.
